We do not accept Medicare/Medicaid policies.

We are out of network with all payers except United Healthcare Choice Plus PPO plans. If your insurance plan is an HMO, out of network providers are not covered. If your plan is a PPO then it will likely provide some level of coverage. Since we do not have knowledge of the details of every insurance plans available, and since every plan is different from state to state, please contact and check with your insurance company/broker to find out if you have acupuncture coverage with your particular plan. 

Filing a claim is not a guarantee of payment.

As a patient, it is your responsibility to ensure you are covered for the services you receive and to provide accurate insurance information to facilitate the billing process. In the case that your own verification has not been done, a superbill can be issued after the office fees have been met.

As a courtesy to you, we will submit claims to these companies on your behalf for companies we’re in-network with only. Patients upon signing new patient agreements, understand that it is the patient’s responsibility to ensure that all co-pays, co-insurance, and deductibles are paid for and met with every visit as detailed in your policy. Services are billed to your insurance based on our Fee Schedule.

Sliding scale, Donation-based, & Pay-What-You-Can visits will not be considered for insurance cases and claims.
